Joining the action at gay men's health: volunteer application form

Thank you for showing an interest in volunteering with Gay Men’s Health. Below you will find an Application Form and an Equal Opportunities Monitoring Form for you to fill in. When you click submit, your application will be sent to me by email. When I receive this I will contact you (normally by telephone) to inform you whether or not you have been selected for interview. In the event that you are, we will arrange an informal interview where you can find out a little more about the organisation and I can find out some more about you and how you would like to be involved.

The information you supply in the application form will be kept strictly confidential. It is intended to provide a record of your contact details and give me an idea of what we can offer you at Gay Men’s Health. The information given on the Equal Opportunities Form is anonymous and allows us to monitor the effectiveness in practice of our Equal Opportunities Policy. The information on your equal opportunities form will be separated from your application and handled by another member of staff. Nonetheless we would be grateful if you could fill it out as fully as you feel is possible or appropriate.
